Welcome to Church View Medical Centre

We are pleased to accept registration requests from patients who are living within the area indicated in the map below:

See our practice boundary and check your postcode is within our catchment area.

 

New Registrations

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

You can register by completing a form available from reception or you can download this form using the link below to print off and complete. Alternatively, anyone wishing to register as a patient can do so by completing this form and emailing it to Churchview.mc@nhs.net, they do not need to register in person at the moment.

Family doctor services registration form

If you are aged 18 years or older we also ask that you fill out a health questionnaire, which are available from reception or can be downloaded using the link below. As with the registration form above, this questionnaire can be emailed to Churchview.mc@nhs.net.
