We are pleased to accept registration requests from patients who are living within the area:
Our surgery can now offer online registration for patients.
Please be aware it takes 8 weeks to register, once you have submitted your forms.
Thank you for applying to register with us here at Church View Medical Centre. With the current pressure on GP services within our area. Our registrations are taking a longer to process.
Please ensure that you continue to order your medications and contact your existing GP practice for your medical needs until you are notified via txt message that you are registered here. We will endeavour of course to process applications as quickly as possible. Please find attached forms below.
